Fitness Center Waiver and Release Agreement

I understand that using the Fitness Center facilities involves voluntary assumption of risk on my part.  I also understand that these risks may result in injury.  Therefore, I agree to take full responsibility for any injuries that may occur conducting my workout at the Indiana Tech Fitness Center.

I also agree to adhere to the following:

1. I understand that it is my responsibility to obtain assistance from the Fitness Center Personnel as pertaining to the following conditions:
   1. If I am not sure how to properly operate/adjust equipment.
      1. If I suspect equipment is damaged, broken, or unsafe.
      1. If I have a question regarding proper lifting technique.
2. I understand that the Indiana Tech Fitness Center STRONGLY urges and recommends all members inform their physician prior to beginning any new fitness program, class, or workout session.
3. Minors are not permitted to use the Fitness Center unsupervised.  Minors ages 16 – 17 may use/participate in the Fitness Center and/or related activities with supervision of a parent/guardian, or while in attendance of an Indiana Tech camp and with the supervision of a camp counselor.  Minors ages 14 – 15 may ONLY use the facility while in attendance of an Indiana Tech camp, and with the supervision of a camp counselor.  This release agreement must to be signed by a parent/guardian prior to using the facility.  Minors under the age of 14 are not permitted in any part of the Fitness Center facility and/or related activities.
4. Upon each arrival to the Fitness Center and/or its related activities I will sign in at the front counter of the Fitness Center.
5. I agree to abide by all Fitness Center rules and policies with reference to the facility, equipment, treatment of employees and fellow patrons, fitness classes and personal training. 

In consideration to Indiana Tech, the undersigned the undersigned does for himself or herself, all heirs, executors successors and assigns, release, waive, discharge and covenant not to sue Indiana Tech, its employees, agents, successors and assigns, of and from any and all actions, causes of action, claims, demands, damages, costs, loss of services, expenses and compensation arising out of, on account of, related to, or in any way connected with the undersigned’s participation in using the Fitness Center facilities or related activities.

The undersigned agrees to all Rules and Regulations set forth by Indiana Tech.
